1. Visit www.emergenetics.com/youth-report to access the questionnaire. Each student must use an ID number that is specific to them.
2. Use the following control codes according to their grade level/graduation year (Please note: only new students who do not already have a Youth Report will need to take the questionnaire):
   1. For 12th Grade Students, graduating in 2025 – Use Control Code uvcsdw
